Athletic Training


The University of Wisconsin-Superior athletic training staff provides a variety of medical services to the university's 20 intercollegiate athletic teams and two club sport programs. 

Our athletic training staff works with physicians, coaches and student athletic trainers to prevent, evaluate, manage and rehabilitate sports injuries.  We work cooperatively with the Essentia Health Sports Medicine Network to ensure the best service to Yellowjacket student-athletes.

**ALL ATHLETIC TRAINING FORMS DUE BY AUGUST 1**

**You will not be allowed to participate if you do not have a physical on file. The physical MUST be completed on the form provided above. We will NOT accept state high school league forms.**

Medical Information Release Authorization Form: This form must be completed and returned to UWS Athletic Training staff if you are requesting any copies of your medical record for any reason.

ADD/ADHD Medication Declaration Information: This form must be completed by your physician and returned to UWS Athletic Training staff if you are taking medications for treatment of ADD/ADHD, as required by the NCAA.

Concussion Protocol: This form outlines that protocol that will be followed by UW-Superior when a student-athlete is suspected of having sustained a concussion.

Mental Health Help: UW-Superior Student Health and Counseling Services offers a variety of services from health education, stress and anxiety coping strategies, along with group and individual counseling services. They are located in MWC 1729.

Visiting Team Information
The Yellowjacket Athletic Training Staff would like to welcome you and your team to Superior, Wisconsin and the University of Wisconsin-Superior. The athletic training room will be open approximately 90 minutes prior to competition and will remain open for 30-60 minutes (or as needed) following competition. Our athletic training room is located within the Marcovich Wellness Center with an entrance in the lobby, across from the welcome desk.  Please use this web page to find pertinent information regarding our staff, services and facilities.

The following will be available upon your visit:
1) Water and cups
2) Injury Ice and bags
3) Modalities (if time & supervision permit): hydrocollator, electrical stimulation, ultrasound
4) Emergency equipment, including AED on-site or within a 3-minute response time
5) Mayo Clinic EMS available within 10-minute response time (depending on fleet availability)
6) Team physician (men's hockey games only)

 
If your team is traveling without a Certified Athletic Trainer:
1) Please notify the UWS Athletic Training Staff prior to your team's visit
2) Electrical modalities will not be provided unless your team travels with a Certified Athletic Trainer or appropriate documentation and instructions are sent with the team.
3) Please ensure that your team travels with the appropriate supplies to meet your taping and wound care needs.
4) Send student-athlete insurance and contact information so that it may be available in the event of an emergency.
